Your specific plan will be unique to your needs and requests
Goals for the first 10 days are focused on attending to basic needs, establishing ongoing community supports, and building resiliency through a sustainable lifestyle of holistic wellness
RCW will accompany you on the following stops, all before arriving at your housing destination:
Check-in with probation officer at courthouse
Pick up prescription medications
Visit DTA office for SNAP benefits, or if they cannot be immediately processed, obtain necessities from local food bank
Redeem clothing voucher
Attend CHC nurse practitioner intake appointment to establish primary care and community based MOUD provider
Spend the afternoon connecting with supportive family or friend that you’ve been looking forward to seeing; if you cannot physically connect with them, brainstorm ways that you can reach out or move closer towards a connection
Attend in-person AA/NA meeting with the goal of acquiring one phone number
Attend CHD intake appointment for behavioral health services
Attend yoga class at the Recover Project
Check-in with RCW via phone call or text
Connect with vocational supports: MassHire, Harmon Temp Agency, Mass Rehab, Career Center, or previous employer
Connect with educational pursuits: Greenfield Community College to enroll in courses; the Literacy Project to complete HiSet
In-person or phone contact with Recovery Coach
Self-check-in: How is my sleeping? Consider sleep hygiene practices to incorporate into your routine, consider asking RCW for guidance if you need suggestions
Meet up for coffee with RCW
Participate in RCW sponsored hike with peers
State agency needs?
DCF worker for service plan clarification
DMH case manager connection
DMV hearing to reinstate license
Acupuncture session with The People’s Medicine Project
Attend RCW facilitated ATAR-Y group
Complete a Committed Action based upon values identified during ATAR-Y group
Inquire about a YMCA subsidized gym membership
Attend CHD Early Recovery group
Dental Appointment
Attend AA/NA meeting and consider what qualities you would like in a sponsor
Contribute to the community with purpose: volunteer at Stone Soup Café, reach out to Jail to Community Task Force to become involved with an upcoming project
Choose an activity from the Daily Maintenance portion of your Wellness and Recovery Action Plan
Engage in a mindfulness practice for 2 minutes
Complete a self-check-in: How are you doing mentally, physically, and spiritually
Meet RCW at Sheriff’s Dept. community garden space
Reflect on the past 10 days with your RCW
Identify what follow-up phone calls/appointments need to be made
Celebrate your successes over the prior days with your RCW
Create schedule for next two weeks
